Agape Long term Care keeps you safe and independent at home across Dallas Fort Worth. We connect you to Texas Medicaid programs PHC, PCS, CAS, CLASS, DBMD, FC, MDCP, and CDS. We offer personal attendant care for bathing and dressing, meals, light housekeeping, grocery support, respite, adaptive aids, and minor home modifications depending on your needs and concerns. We prioritize clear communication, affordability and compassionate during our onboarding process. we screen eligibility, handle paperwork, coordinate with your physician or case manager, and schedule your start of care. Many families are qualified to pay little or nothing out of pocket.

How to Qualify for Long Term Care?
You are eligible to receive our services if you-
Need help with daily tasks such as bathing, dressing, meals, light housekeeping, grocery support, or safe mobility.
We screen you for Texas Medicaid programs PHC, PCS, CAS, CLASS, DBMD, FC, MDCP, and CDS.
We coordinate the physician order or case manager authorization, complete HHSC paperwork, verify benefits, and schedule your start of care.
Need help with daily tasks such as bathing, dressing, meals, light housekeeping, grocery support, or safe mobility.
