Director of Nursing (DON)
POSITION SUMMARY:
Director of Nursing (Registered Nurse with 2 years Home Health experience)
• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current license as a Registered Nurse (RN) in Texas
• Minimum two year of home healthcare experience as a Registered Nurse, experience in home healthcare is strongly preferred. Management experience is strongly preferred.

LVN Intake Coordinator
Position Summary:
- Responsible for the new patient onboarding process for Agape Home Health.
- Accepting referrals through the fax queue, e-mail, and various online referral management systems.
- Reviewing medical documentation for appropriateness of Home Health complete demographics, payor source, history and physical, and orders.
- Entering all referrals into the Medical Records System accurately and timely.
- Contacting the referral source for missing information and communication with the Director of Nursing for additional assistance.
- Maintaining a Pending Log for proper follow through.
- Follow up on any pending referrals with the Director of Nursing and assist them in gathering needed information.
- Completing the Insurance Verification to ensure the patient has Home Health Benefits and input accurate payer sources in EMR.
- Utilizing Insurance verification information in order to request authorization for ordered Home Health services.
- Providing excellent customer service to Referral Sources and Patients
Qualification Guidelines:
- Licensed Vocational Nurse (LVN) with minimum of 3 years clinical experience and at least 1 year home health experience preferred.
- Must be detail-oriented.
- Demonstrate good typing and computer skills.
- Possess excellent interpersonal and communication skills with pleasant & positive phone etiquette.
- Have the ability to work under close supervision and to follow verbal and written instructions.
- Demonstrate good organizational skills and time management skills. Be able to meet or exceed deadlines.
- Be willing to work in an ever-changing healthcare environment and be able to work in a variety of environmental conditions.
- Be able to multi-task and utilize effective problem solving skills.
- Be willing to perform other duties as assigned by the supervisor.
- Basic knowledge of medical terminology.
- Team player with excellent interpersonal communication
- Bilingual (Spanish-preferred)
